The Control of Dogs Order 1992 has specific requirements for pet identification in the UK. The ID Tag must include the surname and address of the owner. This need only be the house name/number and postcode which can fit on one line. Telephone numbers are optional.

Dog owners can be fines up to £5000 if their dog does not wear an ID Tag that complies with the above.
